Roles & Responsibilities
- Provide daily occupational therapy treatments to residents, delivering hands-on, meaningful care
- Document progress notes and maintain accurate, timely clinical records
- Collaborate with the therapy team and facility staff to coordinate care plans
- Achieve and track productivity goals in line with facility expectations
- Follow Continuum Rehabilitation Group policies and facility protocols
- Maintain professional presence on-site to support guaranteed hours and continuity of care
Qualifications
- Certified Occupational Therapy Assistant (COTA) credential required
- 1-3 years of relevant experience; minimum 1 year experience expected
- Prior skilled nursing facility (SNF) experience strongly preferred
- Ability to meet productivity expectations and follow agency and facility policies
- Professional appearance and adherence to dress code (black scrubs required)
- Reliable on-site presence and strong teamwork and communication skills
Knowledge & Skills
- Skilled in delivering daily treatments tailored to SNF residents
- Strong documentation skills for progress notes and clinical records
- Familiarity with SNF workflows and interdisciplinary team collaboration
- Time management and productivity tracking to meet performance targets
- Comfortable following agency policies and facility-specific procedures
