Roles & Responsibilities
- Assess patients’ mobility, strength, and functional abilities and create individualized treatment plans
- Provide hands-on therapy and therapeutic exercises to improve mobility, reduce pain, and restore function
- Document patient progress clearly and timely to support continuity of care and regulatory compliance
- Collaborate with nurses, physicians, and rehab team members to coordinate patient-centered care
- Educate patients and families on home exercise programs and safe mobility techniques
- Participate in discharge planning and goal-setting to maximize patient independence
Qualifications
- Licensed Physical Therapist in Massachusetts or eligible to obtain state licensure
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Experience in post-acute, skilled nursing, or outpatient rehab is preferred
- Strong clinical reasoning, assessment, and manual therapy skills
- Excellent communication skills and a team-oriented mindset
Education
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy
Knowledge & Skills
- Knowledge of therapeutic exercise, gait training, and transfer techniques
- Familiarity with documentation standards and electronic medical records
- Ability to perform patient assessments and set measurable functional goals
- Skilled in adaptive equipment recommendations and fall prevention strategies
- Strong interpersonal skills for patient and family education
- Comfort working in a fast-paced, interdisciplinary skilled nursing environment
