Roles & Responsibilities

• Assess patients’ mobility, strength, and functional abilities and create individualized treatment plans
• Provide hands-on therapy and therapeutic exercises to improve mobility, reduce pain, and restore function
• Document patient progress clearly and timely to support continuity of care and regulatory compliance
• Collaborate with nurses, physicians, and rehab team members to coordinate patient-centered care
• Educate patients and families on home exercise programs and safe mobility techniques
• Participate in discharge planning and goal-setting to maximize patient independence

Qualifications

• Licensed Physical Therapist in Massachusetts or eligible to obtain state licensure
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Experience in post-acute, skilled nursing, or outpatient rehab is preferred
• Strong clinical reasoning, assessment, and manual therapy skills
• Excellent communication skills and a team-oriented mindset

Education

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy

Knowledge & Skills

• Knowledge of therapeutic exercise, gait training, and transfer techniques
• Familiarity with documentation standards and electronic medical records
• Ability to perform patient assessments and set measurable functional goals
• Skilled in adaptive equipment recommendations and fall prevention strategies
• Strong interpersonal skills for patient and family education
• Comfort working in a fast-paced, interdisciplinary skilled nursing environment
