Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents in a long-term care setting
- Administer medications and treatments safely and accurately according to orders
- Perform thorough assessments and document changes in patient condition
- Develop and update individualized care plans in collaboration with the care team
- Communicate clearly with families and interdisciplinary staff to coordinate care
- Maintain a safe environment by following infection control and safety protocols
- Participate in quality improvement activities and contribute to positive outcomes
- Support and mentor nursing assistants and newer staff when appropriate
Qualifications
- Current and active NY state RN or LPN license (as appropriate for role)
- Basic Life Support (BLS/CPR) certification required
- Current immunizations and health screening per facility policy
- Previous experience in long-term care, skilled nursing, or similar setting preferred
- Strong clinical assessment skills, critical thinking, and patient-centered mindset
- Excellent communication and teamwork skills
Education
- Associate Degree in Nursing (ADN) or Nursing Diploma required for RN roles
- Practical Nursing diploma or certificate for LPN roles
- BSN preferred but not required
Knowledge & Skills
- Skilled in medication administration and documentation
- Ability to perform head-to-toe assessments and recognize clinical changes
- Familiarity with infection prevention and safety best practices
- Comfortable using electronic medical records and completing accurate charting
- Strong time management and prioritization in a fast-paced environment
- Effective interpersonal skills for working with residents, families, and staff
