Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a long-term care setting
• Administer medications and treatments safely and accurately according to orders
• Perform thorough assessments and document changes in patient condition
• Develop and update individualized care plans in collaboration with the care team
• Communicate clearly with families and interdisciplinary staff to coordinate care
• Maintain a safe environment by following infection control and safety protocols
• Participate in quality improvement activities and contribute to positive outcomes
• Support and mentor nursing assistants and newer staff when appropriate

Qualifications

• Current and active NY state RN or LPN license (as appropriate for role)
• Basic Life Support (BLS/CPR) certification required
• Current immunizations and health screening per facility policy
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Strong clinical assessment skills, critical thinking, and patient-centered mindset
• Excellent communication and teamwork skills

Education

• Associate Degree in Nursing (ADN) or Nursing Diploma required for RN roles
• Practical Nursing diploma or certificate for LPN roles
• BSN preferred but not required

Knowledge & Skills

• Skilled in medication administration and documentation
• Ability to perform head-to-toe assessments and recognize clinical changes
• Familiarity with infection prevention and safety best practices
• Comfortable using electronic medical records and completing accurate charting
• Strong time management and prioritization in a fast-paced environment
• Effective interpersonal skills for working with residents, families, and staff
