Roles & Responsibilities
- Provide compassionate, patient-centered nursing care in the home setting
- Perform thorough assessments and create individualized care plans
- Administer medications and treatments safely and accurately
- Coordinate care with therapists, social workers, and interdisciplinary team members
- Educate patients and caregivers on condition management and safety
- Document visits and clinical findings clearly and timely in the chart
- Advocate for patient needs and ensure continuity of care across settings
Qualifications
- Active RN license in North Carolina (required)
- Home health or hospice nursing experience (required for nursing roles)
- BLS (Basic Life Support) certification required; other certifications encouraged
- Comfortable performing advanced home-based skills as appropriate for your role
- Excellent communication skills and ability to collaborate with a team
- Ability to perform safe visits in patients' homes and represent CarePartners professionally
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) — BSN preferred
Knowledge & Skills
- Skilled in home health and hospice nursing best practices
- Strong assessment and care planning abilities
- Familiarity with documentation standards and EMR charting
- Patient and family education skills with a compassionate approach
- Time management and route planning for efficient home visits
- Ability to work collaboratively with therapy and support staff
