Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment to residents in a skilled nursing environment
• Develop individualized care plans and measurable goals that promote function and independence
• Deliver hands-on interventions, therapeutic exercise, gait training and use of adaptive equipment
• Document progress accurately and timely in the facility charting system
• Collaborate with nurses, OT, SLP and interdisciplinary team to coordinate resident care
• Educate residents and families on safe mobility, home exercise programs and fall prevention
• Participate in team meetings and contribute to quality improvement initiatives

Qualifications

• Current New Mexico physical therapist license required
• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy
• BLS/CPR certification required; additional certifications a plus
• Experience in skilled nursing facility or geriatrics preferred
• Ability to work Day shifts and complete documentation in a timely manner
• Agencies must have an active NM caregivers account to submit candidates

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy acceptable

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy and gait training
• Strong clinical reasoning and goal-focused treatment planning
• Familiarity with SNF documentation, Medicare and regulatory expectations
• Excellent communication and teamwork skills
• Comfortable with assistive devices, transfers and safe patient handling

This is a contract position in Santa Fe, a cultural gem with amazing food and scenery. Explore the historic Santa Fe Plaza and Canyon Road galleries during your time off, hike the trails in the Sangre de Cristo foothills or take a scenic drive to Bandelier National Monument.
