Roles & Responsibilities

• Evaluate patients and perform thorough assessments to create individualized treatment plans
• Deliver hands-on therapy and evidence-based interventions to improve mobility and function
• Document progress and update care plans in the medical record accurately and timely
• Collaborate with interdisciplinary teams, family members, and caregivers to optimize outcomes
• Educate patients and caregivers on exercise programs, safety, and fall prevention
• Lead discharge planning and recommendations to ensure safe transitions home
• Maintain a safe therapeutic environment and adhere to regulatory and facility standards

Qualifications

• Current Physical Therapist license in California (required)
• Doctor of Physical Therapy (DPT) or Master’s/Bachelor’s in Physical Therapy
• Basic Life Support (BLS) certification (current)
• Active malpractice coverage or ability to obtain prior to start (per facility policy)
• Professional, patient-centered demeanor with strong communication and teamwork skills
• Previous outpatient or inpatient rehab experience preferred

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy accepted

Knowledge & Skills

• Skilled in musculoskeletal and neurologic rehab techniques
• Proficient with mobility training, therapeutic exercise, manual therapy, and modalities
• Comfortable with patient documentation and electronic health record systems
• Strong clinical reasoning, assessment, and treatment planning abilities
• Ability to train and educate patients and caregivers effectively
• Good time management and prioritization in a fast-paced rehab setting
