Roles & Responsibilities

Deliver personalized physical therapy to patients recovering from falls and fractures
Complete OASIS assessments and Start of Care evaluations with accuracy and compassion
Travel across four rural counties, bringing expert care directly to patients' homes
Collaborate with an interdisciplinary team to create meaningful recovery plans
Track visits and time seamlessly using provided tablet technology

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working independently in rural community settings
Reliable transportation to serve multiple counties

Knowledge & Skills

Strong assessment and clinical reasoning skills
Excellent time management for 4-6 patient visits daily
Clear communication with elderly patients and their families
Tech-comfortable for tablet-based documentation
