Roles & Responsibilities

• Evaluate patients across inpatient, swing bed, outpatient, and skilled nursing settings to determine functional status and therapy needs.
• Develop individualized treatment plans based on physician orders, clinical reasoning, and patient goals.
• Provide skilled physical therapy interventions and progress patients toward measurable outcomes.
• Document assessments, plans, and progress accurately and timely in the EMR to ensure continuity of care.
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate comprehensive patient care.
• Educate patients and caregivers on home programs, safety, and strategies to maximize independence.
• Manage diverse patient populations and complex diagnoses with professionalism and attention to detail.
• Engage in ongoing professional development to maintain best practices and compliance with state and federal PT regulations.

Qualifications

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program
• Active California Physical Therapy License in good standing
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA Basic Life Support (BLS) certification
• Minimum 1 year experience in acute care, swing bed/skilled nursing, or outpatient settings
• Proficiency with EMR systems and basic computer skills
• Strong clinical reasoning, communication, documentation, and problem-solving abilities
• Ability to manage diverse patient populations and adhere to California and federal PT regulations

Education

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program
