Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start of care assessments, recertifications, and resumption of care visits
Deliver 6.5 points of care per day while maintaining high-quality patient outcomes
Collaborate with clinical coordinators and directors for guidance and support
Perform safety evaluations and teaching visits in hospice situations when needed
Document patient progress using OASIS and maintain accurate clinical records

Qualifications

Current ND Physical Therapy License required
BLS certification required
Valid driver's license and auto insurance required
Home Health experience preferred
OASIS training preferred
First-time travelers welcome to apply

Knowledge & Skills

Physical Therapy expertise in home health settings
Strong assessment and clinical decision-making abilities
Excellent communication and patient education skills
Ability to work independently while staying connected with the care team
Comfortable with documentation systems and OASIS requirements
