Roles & Responsibilities

• Conduct comprehensive home health evaluations and establish individualized treatment plans
• Provide skilled physical therapy interventions during in-home visits to improve mobility and independence
• Document all visits and clinical findings thoroughly in HCHB to maintain continuity of care
• Coordinate care with interdisciplinary team members, including nursing and hospice when applicable
• Educate patients and caregivers on safe home exercise programs and fall prevention strategies
• Manage a caseload efficiently while maintaining patient-centered, high-quality outcomes
• Ensure timely scheduling and travel planning for patient visits; reliable vehicle recommended

Qualifications

• Minimum 2 years of relevant experience (job listing indicates 'Experience: 2') with home health therapy preferred
• Demonstrated home health experience for therapy positions (explicitly requested in the hiring profile)
• Comfortable delivering care in patients' homes and adapting treatment plans to home environments
• Strong documentation skills with familiarity or willingness to use HCHB
• Reliable vehicle for patient visits (noted as needed in the job details)

Knowledge & Skills

• Proficient in home health physical therapy practices and patient education
• Experience with HCHB documentation system
• Excellent time management and scheduling skills for multiple daily patient visits
• Ability to coordinate with nursing and hospice teams when needed
• Skilled in developing practical, home-based treatment plans and fall-prevention strategies
