Roles & Responsibilities

Provide compassionate, high-quality nursing care to residents and patients
Administer medications and treatments accurately and on schedule
Monitor, assess and document patient conditions and changes
Collaborate with nurses, providers and the care team to support patient plans
Educate residents and families about care routines and health needs
Maintain infection control, safety protocols and a clean care environment
Assist with admissions, discharges and transfers smoothly and professionally
Keep precise, timely clinical documentation in the patient record

Qualifications

Current LPN license in Pennsylvania (required)
Practical Nursing diploma or equivalent (completed LPN/Vocational program)
Prior LPN experience preferred but new grads with strong clinical skills are welcome
Current BLS/CPR certification preferred or willing to obtain
Ability to pass background check and drug screen as required by facility
Strong communication skills and a patient-centered attitude

Education

Practical Nursing diploma or equivalent (LPN/Vocational)

Knowledge & Skills

Medication administration and charting accuracy
Basic wound care and dressing changes
Proficient in taking and interpreting vital signs
Comfortable with electronic charting / clinical documentation
Excellent teamwork, time management and prioritization
Patient advocacy and compassionate bedside manner
