Roles & Responsibilities

Provide direct patient care with compassion and professionalism
Administer medications and treatments as directed, following facility protocols
Monitor and document patient status, reporting changes to the care team promptly
Assist with activities of daily living and ensure patient comfort and safety
Collaborate with nurses and interdisciplinary staff to deliver coordinated care
Maintain accurate and timely electronic charting and handoffs

Qualifications

Active LPN license (practical nursing) — Pennsylvania licensure preferred
Completion of an accredited practical nursing program or vocational nursing diploma
Current BLS/CPR certification (or willingness to obtain)
Ability to pass background check and meet facility vaccination/immunization policies
Strong communication skills and a team-first attitude

Education

Vocational Nursing Diploma or Practical Nursing certificate (required)

Knowledge & Skills

Medication administration and accurate documentation
Basic patient assessment and vital sign monitoring
Familiarity with electronic health record charting
Infection control and safe patient handling practices
Professional communication with patients, families, and interdisciplinary teams
