Roles & Responsibilities

• Provide compassionate, hands-on care to residents and patients, assisting with activities of daily living
• Take and record vital signs and report changes in condition promptly
• Assist with transfers, mobility and safe ambulation to promote independence
• Maintain accurate, timely documentation of care and observations
• Follow infection control, safety protocols and facility policies to protect patients and staff
• Collaborate with nurses and the care team to support individualized care plans
• Build trusting relationships with patients and their families through clear, respectful communication

Qualifications

• Current CNA certification or state equivalent (PA certification preferred)
• Valid work authorization to practice in Pennsylvania
• Comfortable working in a fast-paced healthcare setting with strong attention to detail
• Ability to pass background check and required health screenings (TB, immunizations)
• Reliable, punctual, and a strong team player who puts patient safety first
• Previous experience in long-term care or skilled nursing is a plus

Education

• High school diploma or equivalent
• CNA certification

Knowledge & Skills

• Basic patient care and activities of daily living (ADLs)
• Accurate vital signs measurement and documentation
• Safe transfer and mobility assistance techniques
• Infection prevention and proper PPE use
• Clear verbal and written communication with care teams and families
• Time management and the ability to prioritize patient needs
• Familiarity with electronic charting or readiness to learn
