Roles & Responsibilities

Provide compassionate, competent nursing care to residents as an LPN, following care plans and facility policies
Administer medications and treatments safely and document accurately
Monitor resident conditions and communicate changes promptly to the care team
Support activities of daily living and promote resident dignity and independence
Collaborate with nurses, therapists, and family members to deliver personalized care
Participate in charting, resident assessments, and handoffs to ensure continuity of care

Qualifications

Valid Licensed Practical Nurse (LPN) license in Pennsylvania (required)
Current BLS/CPR certification (or willing to obtain upon hire)
Comfortable with medication administration, wound care basics, and documentation
Strong communication skills and a team-first attitude
Previous long-term care or skilled nursing experience preferred (1–2 years preferred)

Education

Graduate of an accredited LPN/Licensing Practical Nurse program

Knowledge & Skills

Knowledge of basic nursing procedures, infection control, and safe medication administration
Skilled in resident assessment, reporting changes, and following care plans
Familiarity with electronic or paper charting and accurate documentation
Ability to work collaboratively with interdisciplinary teams
Compassionate bedside manner and strong patient advocacy skills
