Roles & Responsibilities

Provide compassionate direct resident care including bathing, dressing, and activities of daily living
Administer medications and treatments accurately and on time
Monitor and document vital signs, changes in condition, and care delivered
Collaborate with RNs, therapists, and the interdisciplinary team to implement care plans
Educate residents and families about care plans and medications in a clear, respectful way
Maintain a safe, clean environment and follow infection prevention practices
Accurately complete required clinical documentation and handoffs
Respond promptly to resident needs and emergent situations

Qualifications

Current Licensed Practical Nurse (LPN) license in the appropriate jurisdiction (CT license required or eligibility)
Basic Life Support (BLS/CPR) certification (or willingness to obtain)
Comfortable working in a long-term care/skilled nursing environment
Strong communication skills and a team-first attitude
Reliable, professional, and patient-centered approach to care

Education

Graduate of an accredited Practical Nursing program or equivalent LPN training

Knowledge & Skills

Medication administration and documentation
Wound care basics and skin integrity monitoring
Vital signs assessment and clinical observation
Electronic health record (EHR) charting familiarity
Time management and prioritization during busy shifts
Clear, empathetic communication with residents and families
