Roles & Responsibilities

• Provide compassionate, skilled physical therapy interventions appropriate for a skilled nursing facility population
• Assist with patient assessments, mobility training, transfers, gait and balance programs
• Collaborate closely with nurses, therapists and interdisciplinary team members to optimize patient outcomes
• Complete timely, accurate documentation and contribute to patient care plans
• Adhere to Continuum Rehabilitation Group policies and facility procedures
• Maintain productivity standards (facility notes a 92% productivity expectation)
• Represent the therapy team professionally (black scrubs dress code) and be present onsite to receive guaranteed hours

Qualifications

• 1–3 years of relevant PTA experience (as noted in the job posting)
• Credentialed as a PTA per facility/state requirements (have documentation ready)
• Experience working in a skilled nursing facility (SNF) or similar post-acute care setting preferred
• Ability to follow facility policies and meet established productivity expectations
• Reliable, able to be present onsite during scheduled hours
