Roles & Responsibilities

• Provide comprehensive inpatient pharmacy care including accurate order entry and verification
• Check and dispense medications ensuring the highest medication safety standards
• Counsel patients and communicate effectively with providers, nurses, and care teams
• Rotate into ambulatory services as assigned, such as anticoagulation clinic or oncology
• Support care for diverse patient populations from pediatrics to geriatrics
• Collaborate with pharmacy technicians and other staff to optimize workflow and patient outcomes
• Adhere to state and federal pharmacy laws and institutional policies to maintain compliance

Qualifications

• Current Pennsylvania pharmacist license or eligible for reciprocation within 90 days of hire
• Bachelor of Science in Pharmacy required; Doctor of Pharmacy degree preferred
• Recent hospital pharmacy experience preferred
• Pharmacy residency preferred if rotating to ambulatory services
• Advanced practice certifications preferred for specialized ambulatory roles
• Basic Life Support (BLS) certification required for anticoagulation clinic rotation and renewable every two years
• Strong interpersonal skills and ability to work collaboratively with multidisciplinary teams

Education

• Bachelor of Science in Pharmacy (required)
• Doctor of Pharmacy (PharmD) preferred

Knowledge & Skills

• Medication safety and error prevention
• Order entry and verification workflows
• Medication checking and dispensing best practices
• Patient counseling and interprofessional communication
• Experience in inpatient pharmacy operations and ambulatory clinic services
• Familiarity with state and federal pharmacy regulations
