Roles & Responsibilities

Provide compassionate, safe patient care consistent with LPN scope of practice
Administer medications and treatments accurately and document thoroughly
Monitor patient condition, report changes to the RN or provider, and follow care plans
Collaborate with the care team to ensure continuity and quality of care
Maintain accurate and timely charting in the facility’s documentation system
Support patient and family education with clarity and empathy

Qualifications

Valid LPN license in Pennsylvania (required)
Current basic life support (BLS/CPR) certification preferred
Comfortable working in a per diem capacity and adapting to different unit needs
Ability to pass required background checks and facility screenings
Strong communication skills and a team-oriented mindset

Education

Graduate of an accredited practical nursing program or equivalent diploma

Knowledge & Skills

Solid practical nursing skills including medication administration and wound care
Proficiency with patient assessments and monitoring vital signs
Good charting practices and attention to documentation detail
Patient-centered communication and the ability to work collaboratively
Time management and prioritization during busy shifts
