Roles & Responsibilities

Provide compassionate, patient-centered care with attention to safety and dignity
Administer medications and treatments accurately and according to policy
Perform routine assessments and document patient status in the chart
Collaborate with the interdisciplinary team to deliver coordinated care
Assist with activities of daily living and patient mobility when needed
Communicate clearly with families and teammates about care plans and changes

Qualifications

Valid LPN licensure (Pennsylvania license required or eligibility to obtain)
Current basic life support (BLS) certification as required by facility
Comfortable with medication administration, wound care basics, and charting
Professional, reliable, and able to work well within a team
Prior LPN experience preferred but enthusiastic new grads may be considered

Education

LPN certificate or diploma
High school diploma or GED

Knowledge & Skills

Strong clinical assessment and observation skills
Medication administration and documentation proficiency
Basic knowledge of infection control and patient safety practices
Familiarity with electronic health records or willingness to learn
Good communication and teamwork skills
