Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments according to care plans and facility policy
Accurately document patient status, interventions, and responses in charts
Collaborate with nurses and interdisciplinary team to deliver coordinated care
Monitor and report changes in patient condition to supervising clinicians
Support admissions, transfers, and daily activities while maintaining patient dignity
Uphold infection control, safety standards, and regulatory compliance

Qualifications

Active LPN license (Licensed Practical Nurse) — required
License valid in Pennsylvania (or ability to obtain prior to start)
Basic Life Support (BLS/CPR) certification preferred
Comfortable with medication administration, charting, and resident care
Strong communication skills and the ability to work within a care team
Previous experience as an LPN preferred but new grads may inquire

Education

LPN diploma or certificate from an accredited program

Knowledge & Skills

Medication administration and documentation
Basic patient assessment and vital signs
Electronic charting and clinical documentation
Infection prevention and safety procedures
Time management and prioritization in a busy care setting
Team communication and resident-centered care
