Roles & Responsibilities

Provide compassionate bedside care and daily living assistance to residents
Administer medications and treatments accurately and timely
Monitor and document patient conditions, vital signs, and changes in status
Collaborate with nurses and the interdisciplinary team to support care plans
Assist with admissions, discharges, and transfers as needed
Communicate clearly with residents and families, offering education and reassurance
Maintain clean, safe, and compliant documentation in the medical record

Qualifications

Current, active LPN license in Pennsylvania (required)
Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
Ability to pass background check and meet facility immunization requirements
Strong clinical assessment skills and attention to detail
Compassionate approach to resident-centered care and good communication skills

Education

Completion of an accredited LPN program or vocational nursing diploma
High school diploma or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Basic nursing procedures and wound care fundamentals
Proficiency with electronic medical records or willingness to learn
Ability to perform reliable patient assessments and report changes
Infection control standards and safe patient handling techniques
Teamwork, time management, and effective communication
