Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with care plans
• Administer medications and treatments accurately and on schedule
• Monitor and document patient status, vital signs, and changes in condition
• Collaborate with nurses, therapists, and interdisciplinary team members to coordinate care
• Support resident and family education with clear, patient-focused communication
• Assist with admissions, transfers, and discharges to ensure smooth transitions
• Maintain accurate clinical documentation and follow facility policies and procedures
• Promote a safe, clean environment and adhere to infection control standards

Qualifications

• Valid Licensed Practical Nurse (LPN) license (ability to practice in Pennsylvania preferred)
• Current CPR/BLS certification or willingness to obtain
• Comfortable administering medications and performing routine nursing procedures
• Strong communication skills and a team-oriented mindset
• Prior experience in long-term care or skilled nursing preferred but not required

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and documentation
• Patient assessment and basic clinical skills
• Familiarity with electronic medical records and charting
• Infection control and resident safety practices
• Time management, organization, and reliable follow-through
• Compassionate patient- and family-centered communication
