Roles & Responsibilities

• Provide compassionate basic nursing care to residents in line with care plans
• Administer medications and treatments safely and accurately per policy
• Monitor and document patient status, reporting changes to the care team
• Collaborate with nurses, CNAs and interdisciplinary team members to deliver coordinated care
• Educate residents and families about care plans and daily routines
• Maintain accurate and timely charting and documentation
• Support admissions, transfers and discharges to ensure smooth transitions
• Adhere to infection control standards and facility safety protocols

Qualifications

• Active LPN license (practical/vocational nursing) — ability to practice in Pennsylvania preferred
• Current BLS/CPR certification (American Heart Association or equivalent) preferred
• Completion of an accredited LPN program
• Clinical experience in long-term care, skilled nursing, or similar settings preferred
• Strong communication skills and ability to work as part of a team
• Ability to pass required background checks and health screenings

Education

• High school diploma or GED and completion of an accredited LPN program

Knowledge & Skills

• Medication administration and documentation
• Vital signs assessment and basic clinical observations
• Patient-centered communication and family education
• Time management and prioritization in a fast-paced setting
• Basic wound care and assistance with activities of daily living
• Familiarity with electronic medical records and accurate charting
