Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients with professionalism and warmth
Administer medications and treatments accurately and document all care thoroughly
Monitor and record vital signs, recognize changes in condition, and escalate when needed
Assist with daily living activities while promoting independence and dignity
Collaborate with the interdisciplinary care team to create and follow individualized care plans
Maintain accurate, timely charting and communicate clearly with families and team members

Qualifications

Current, active LPN license (practical/vocational nursing) valid in Pennsylvania
Basic Life Support (BLS) certification (or willingness to obtain upon hire)
Previous LPN clinical experience preferred; newly graduated LPNs are welcome to apply and grow
Strong communication skills and ability to work as part of a team
Reliable, patient-focused, and able to follow facility policies and protocols

Education

LPN diploma or equivalent from an accredited program

Knowledge & Skills

Skilled in medication administration and basic nursing procedures
Comfortable with vital sign monitoring, wound care basics, and ADL assistance
Familiarity with electronic medical records (EMR) systems is a plus
Good critical thinking, documentation, and time-management abilities
Personable bedside manner and strong teamwork/collaboration skills
