Roles & Responsibilities

Provide compassionate bedside care to residents and patients, making each day safer and more comfortable
Administer medications and treatments accurately and timely according to orders
Monitor and document vital signs, patient status, and progress in the medical record
Collaborate with RNs and the interdisciplinary team to coordinate care plans
Educate patients and families about care, medications, and follow-up needs in a clear, reassuring way
Maintain accurate, timely documentation and report changes in condition promptly

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain upon hire)
Prior experience in long-term care, rehab, or medical-surgical settings is a plus
Strong communication skills and a patient-centered attitude
Ability to pass background checks and meet facility onboarding requirements

Education

Completed Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

Medication administration and documentation best practices
Basic patient assessment and monitoring (vitals, skin integrity, mobility)
Infection control and safe patient handling techniques
Familiarity with electronic health records and accurate charting
Effective teamwork and time management in a fast-paced clinical setting
