Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents as an LPN
- Administer medications and document accurately according to policy
- Monitor patient conditions and report changes to RN or provider
- Assist with ADLs and promote resident dignity and independence
- Collaborate with the care team to create and follow care plans
- Maintain clear, timely charting and communicate with families
Qualifications
- Current LPN license in good standing (PA license preferred)
- Current CPR/BLS certification or willingness to obtain
- Ability to pass background check and required health screenings
- Strong communication and teamwork skills
- Previous long-term care or acute care experience preferred but not required
Education
- Diploma or certificate from an accredited LPN/Vocational Nursing program
- Current state LPN license
Knowledge & Skills
- Familiarity with medication administration and documentation
- Basic wound care and vital signs monitoring
- Comfortable using electronic charting systems
- Good observation skills and clinical judgment
- Patient-centered approach and strong interpersonal skills
