Roles & Responsibilities

• Provide compassionate, safe nursing care to residents in accordance with facility policies
• Administer medications and treatments accurately and on time
• Monitor patient status and promptly report changes to the care team
• Document assessments, interventions, and outcomes clearly in the medical record
• Collaborate with nurses, therapists, and providers to deliver coordinated care
• Support residents' activities of daily living with dignity and respect
• Maintain infection control standards and adhere to safety protocols
• Participate in shift handoffs to ensure continuity of care

Qualifications

• Current Licensed Practical Nurse (LPN) license valid in Pennsylvania
• Completion of an accredited practical nursing program
• Current BLS/CPR certification (or willing to obtain prior to start)
• Ability to pass required background checks and health screenings
• Comfortable working in a fast-paced long-term care setting and adaptable to changing schedules
• Strong communication skills and a team-first attitude

Education

• Completion of an accredited LPN (Practical Nursing) program
• High school diploma or GED

Knowledge & Skills

• Clinical assessment and basic patient monitoring
• Medication administration and documentation
• Wound care fundamentals and infection prevention
• Familiarity with electronic medical records and charting
• Time management, prioritization, and teamwork
• Compassionate resident-centered care and excellent bedside manner
