Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents
Administer medications and treatments safely and accurately
Monitor resident condition, document observations, and report changes
Collaborate with nurses, therapists, and care team members on care plans
Assist with activities of daily living and support resident independence
Promote a safe, clean environment and follow infection-control protocols

Qualifications

Current LPN license in the state of Connecticut (required)
BLS/CPR certification required or willingness to obtain upon hire
Experience in long-term care or skilled nursing preferred
Strong clinical assessment, communication, and documentation skills
Ability to work effectively as part of an interdisciplinary care team

Education

Graduate of an accredited practical nursing (LPN/LVN) program

Knowledge & Skills

Knowledge of medication administration and nursing best practices
Familiarity with electronic medical record (EMR) charting systems
Skills in wound care, ADL assistance, and basic nursing procedures
Good infection control practices and resident safety awareness
