Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and assist with daily living activities
Administer medications and treatments according to care plans and facility protocols
Monitor patient status and communicate changes promptly to the care team
Document accurately and timely in the patient chart to ensure continuity of care
Collaborate with nurses, physicians, and interdisciplinary team members to deliver quality care
Support a positive patient experience by treating residents with dignity and respect

Qualifications

Current, active LPN license valid in Pennsylvania
Basic Life Support (BLS) certification preferred or willingness to obtain
Comfortable administering medications and performing routine clinical procedures
Compassionate, dependable, and able to work collaboratively on a care team
Previous LPN experience in long-term care, skilled nursing, or similar setting preferred

Education

Practical Nursing diploma or certificate from an accredited program

Knowledge & Skills

Strong patient assessment and observation skills
Medication administration and safe documentation practices
Familiarity with basic IV/medication protocols common to LPN scope
Good communication skills with patients, families, and team members
Ability to work in fast-paced environments while maintaining attention to detail
