Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents according to plan of care
• Administer medications and treatments accurately and safely
• Perform thorough client assessments and timely documentation
• Communicate clearly with patients, families, and the interdisciplinary team
• Monitor and report changes in patient condition to appropriate providers
• Support a positive, restorative environment focused on dignity and comfort
• Assist with admissions, discharges, and transitions of care as needed
• Follow infection control protocols and facility policies to protect patients and staff

Qualifications

• Current LPN license (Licensed Practical Nurse) — Pennsylvania licensure required or eligible
• Basic Life Support (BLS) / CPR certification (current) preferred
• Current immunizations and TB testing as required by facility
• Ability to pass background and health screenings required by the employer
• Comfortable with electronic charting and basic computer skills
• Clinical experience in a long-term care or similar setting preferred but not required

Education

• Practical Nursing diploma or LPN certificate from an accredited program

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Clear, compassionate communication with residents and families
• Accurate charting and documentation practices
• Time management and ability to prioritize in a fast-paced setting
• Basic wound care and ADL support knowledge
• Teamwork, reliability, and professional integrity
