Roles & Responsibilities

• Provide compassionate direct patient care, promoting dignity and comfort
• Administer medications and treatments accurately and safely
• Perform routine assessments and document findings clearly in the patient chart
• Collaborate closely with RNs, therapists, and interdisciplinary team members to optimize care plans
• Monitor patient status and promptly report changes to the appropriate clinical staff
• Assist with activities of daily living and support patients' functional goals
• Educate patients and families on care plans, medications, and follow-up needs
• Maintain compliance with infection control practices and facility policies

Qualifications

• Current, unencumbered LPN license in Pennsylvania (required)
• Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Diploma or certificate in Practical Nursing (or equivalent)
• Ability to pass background check, drug screen, and required immunizations
• Comfort working in a team-oriented care environment with strong communication skills
• Reliable, compassionate, and patient-focused approach to care

Education

• Diploma or certificate in Practical Nursing (LPN) or equivalent

Knowledge & Skills

• Strong clinical assessment and vital sign monitoring skills
• Medication administration and accurate documentation practices
• Familiarity with basic wound care and infection control
• Patient-centered communication and family education
• Time management and prioritization in a fast-paced setting
• Team collaboration and clear handoff/reporting techniques
• Basic computer/EMR charting skills
