Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and document all care thoroughly
• Monitor resident condition and communicate changes promptly to the care team
• Support resident mobility, activities of daily living, and dignity with respect
• Collaborate with nurses, nursing assistants, and management to deliver coordinated care
• Participate in care planning, including charting and reporting observations
• Assist with admissions, transfers, and discharges when needed to ensure smooth transitions

Qualifications

• Current LPN (Licensed Practical Nurse) license in Pennsylvania or compact/eligible per state rules
• Basic Life Support (BLS) certification required; ACLS/other certifications a plus
• Prior experience in a skilled nursing or long-term care environment preferred
• Demonstrated ability to follow protocols, chart accurately, and communicate effectively
• Ability to pass background checks and meet facility onboarding requirements

Education

• Graduate of an accredited Practical Nurse program or equivalent

Knowledge & Skills

• Strong medication administration and documentation skills
• Knowledge of infection control and safety protocols
• Comfortable with resident assessments and reporting clinical changes
• Good interpersonal skills and the ability to work well on a multidisciplinary team
• Time management and prioritization in a fast-paced care setting
