Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and safely
Perform routine assessments and document changes in condition in the medical record
Collaborate with the interdisciplinary team to create and follow individualized care plans
Communicate clearly and empathetically with residents and families
Monitor and report clinical changes promptly to ensure excellent outcomes
Support a safe, clean environment and follow infection control protocols

Qualifications

Active LPN (Licensed Practical Nurse) license in Pennsylvania (or compact/eligible where applicable)
Current CPR/BLS certification
Comfortable administering medications and performing basic treatments
Strong interpersonal and communication skills; resident-focused attitude
Previous experience in long-term care, skilled nursing, or similar settings preferred
Ability to pass facility background checks and meet onboarding requirements

Education

Practical Nursing diploma or equivalent from an accredited program

Knowledge & Skills

Medication administration and documentation best practices
Basic nursing assessments and vital signs monitoring
Familiarity with electronic medical records and charting
Infection control and safety procedures
Time management and prioritization in a busy clinical environment
Teamwork and effective communication with care teams and families
