Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate day-to-day nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and document thoroughly
Monitor and report changes in patient condition promptly to the care team
Collaborate with RNs, therapists, and support staff to create and follow individualized care plans
Assist with activities of daily living while promoting resident dignity and independence
Maintain accurate clinical records and complete required documentation in a timely manner
Participate in team huddles and contribute to a positive, resident-centered environment

Qualifications

Active LPN license in Pennsylvania (or compact/state-appropriate) — required
Current BLS/CPR certification (or willingness to obtain upon hire)
Previous experience in long-term care or skilled nursing preferred
Strong communication and teamwork skills; a patient-first attitude
Ability to pass background checks and meet facility onboarding requirements

Education

LPN diploma or practical nursing certificate from an accredited program
High school diploma or GED required

Knowledge & Skills

Medication administration and basic wound care skills
Understanding of infection control and standard precautions
Comfortable with electronic charting and clinical documentation
Ability to assess basic clinical signs and escalate concerns appropriately
Time management skills to prioritize resident care effectively
