Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and document per facility policy
Assess resident condition, report changes to the care team, and participate in care planning
Assist with activities of daily living and promote resident comfort and dignity
Collaborate with RNs, CNAs and interdisciplinary team members to deliver high-quality care
Maintain accurate and timely clinical documentation and adhere to safety protocols

Qualifications

Active LPN license (Pennsylvania) or eligibility to obtain one — required
CPR/BLS certification (or willingness to obtain upon hire)
Comfortable with medication administration, basic wound care, and resident assessments
Strong communication skills and a team-oriented attitude
Prior experience in long-term care or skilled nursing is a plus

Education

Graduate of an accredited Practical Nursing program or equivalent diploma/certificate

Knowledge & Skills

Medication administration and documentation
Resident assessment and basic clinical monitoring
Infection control and safety practices
Basic wound care and ADL assistance
Charting/EHR familiarity and attention to detail
Empathy, patience, and strong interpersonal skills
