Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients, treating each person with dignity
• Administer medications and treatments accurately and safely
• Assess and monitor patient condition, reporting changes promptly to the care team
• Document care activities clearly and timely in the patient record
• Support patient comfort and activities of daily living with respect and empathy
• Collaborate with RNs, CNAs and interdisciplinary team members to deliver coordinated care
• Participate in care planning and communicate effectively with families and staff

Qualifications

• Valid Licensed Practical Nurse (LPN) license in Pennsylvania (PA) required
• Current Basic Life Support (BLS/CPR) certification recommended
• Completion of an accredited practical nursing program
• Prior clinical experience in long-term care, skilled nursing, or similar setting preferred
• Strong communication, time management and teamwork skills
• Ability to pass standard background checks and health screenings

Education

• Graduate of an accredited Practical Nursing program
• High school diploma or GED
• Current LPN licensure for Pennsylvania

Knowledge & Skills

• Medication administration and safe handling practices
• Basic wound care and infection control principles
• Vital signs monitoring and clinical assessment
• Accurate charting and documentation skills
• Patient-centered care and effective communication with residents and families
• Ability to work efficiently in a fast-paced, team-based environment
