Roles & Responsibilities

Provide compassionate direct patient care while following facility policies
Administer medications and treatments accurately and document thoroughly
Monitor patient status and communicate changes to the interdisciplinary team
Assist with activities of daily living and support patients' comfort
Participate in care planning and coordinate safe transitions for patients
Maintain accurate and timely clinical documentation and handoffs
Adhere to infection control standards and promote a safe environment

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania or eligibility to obtain one
Current Basic Life Support (BLS) certification or willingness to obtain
Completion of an accredited practical nursing program
Strong clinical assessment and medication administration skills
Excellent communication and teamwork abilities
Reliable, professional, and patient-centered approach to care

Education

Graduation from an accredited Practical Nursing (LPN) program
High school diploma or GED

Knowledge & Skills

Solid understanding of basic nursing procedures and standards of care
Medication administration and documentation proficiency
Vital signs monitoring and basic patient assessment
Familiarity with electronic health records and charting best practices
Ability to prioritize tasks and manage time during shifts
Compassionate patient interaction and effective communication skills
