Roles & Responsibilities

• Provide compassionate, hands-on nursing care to adult and geriatric patients on a med surg unit
• Administer medications and treatments safely and accurately
• Monitor and document patient condition, reporting changes promptly
• Collaborate with nurses, providers, and support staff to deliver coordinated care
• Assist with admissions, discharges, and smooth patient transitions
• Implement safety measures and fall prevention for vulnerable patients
• Educate patients and families about care plans and follow-up
• Maintain accurate electronic charting and complete required documentation

Qualifications

• Active Licensed Practical Nurse (LPN) license in New York
• Experience or comfort working with adult and geriatric populations
• Willingness to work 12-hour shifts and night shifts as needed
• Current Basic Life Support (BLS) certification or willingness to obtain
• Ability to pass facility-required background check and drug screen
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited LPN program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and basic clinical assessments
• Geriatric care principles and adult med surg experience
• Accurate charting in electronic medical records
• Infection control and safe patient handling
• Time management and prioritization on a busy unit
• Collaborative teamwork and clear communication
