Roles & Responsibilities

• Conduct comprehensive mental health and psycho-social assessments for patients and families
• Develop, document, and implement individualized treatment plans in the EHR
• Provide crisis counseling and immediate intervention for patients in distress
• Coordinate discharge planning and link patients to appropriate home and community services
• Manage an assigned caseload and ensure timely, regulatory-compliant documentation
• Collaborate in interdisciplinary team meetings and patient care conferences
• Advocate for patients with community, state, and federal agencies as needed
• Provide education, coaching, and resource referrals to support long-term recovery
• Respond to referrals and consults from primary care and outlying clinics within scope
• Support SEARHC’s mission by contributing to departmental goals and quality initiatives

Qualifications

• Alaska LCSW required at time of submittal (LMSW acceptable in some cases—verify eligibility)
• Minimum 2–4 years of social work experience in inpatient or outpatient healthcare
• Must be at least 21 years of age
• NABCS and ICPA clearances required
• Flu vaccine required prior to start
• Ability to schedule and submit fingerprint cards promptly (plan to mail within 2–3 days)
• Demonstrated excellence in mental health evaluations, crisis counseling, and documentation
• Competence with Electronic Health Records and standard clinical technology

Education

• Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)

Knowledge & Skills

• Strong case management and discharge planning skills
• Proven ability with psycho-social assessments and treatment planning
• Excellent written and verbal communication; clear, timely documentation
• Empathic listening, conflict resolution, and customer service/recovery skills
• Familiarity with community resources and care coordination with multi-disciplinary teams
• Basic nutrition and tobacco cessation knowledge to support physical health goals
• Comfort using EHRs, Google Drive, fax and phone systems for clinical workflows
• Ability to maintain confidentiality and meet regulatory compliance
