Roles & Responsibilities

• Provide skilled occupational therapy services under the direction of the OT to help residents regain independence
• Develop and implement individualized treatment plans with measurable goals
• Perform therapeutic activities, adaptive training, and functional assessments
• Document progress, update care plans, and communicate changes to interdisciplinary team members
• Educate residents and families on strategies to maximize daily function and safety
• Collaborate with nursing and rehab staff to ensure continuity of care and timely interventions

Qualifications

• Certified Occupational Therapy Assistant (COTA) credential — required
• Current state licensure or eligibility to obtain COTA licensure in Texas
• Minimum clinical experience in skilled nursing, long-term care, or rehab settings preferred
• Strong documentation skills and familiarity with rehab care plans
• Current CPR/BLS certification (or willingness to obtain upon hire)

Education

• Associate degree or diploma from an accredited Occupational Therapy Assistant program

Knowledge & Skills

• Skilled in therapeutic techniques for mobility, ADLs, and adaptive equipment
• Excellent interpersonal skills for working with residents, families, and staff
• Knowledge of Medicare/Medicaid documentation and rehab compliance expectations
• Ability to assess functional status and translate findings into effective treatment plans
• Good time management and prioritization in a busy skilled nursing environment
