Roles & Responsibilities

• Perform and document patient assessments appropriate to LPN licensure, ensuring accuracy and timeliness
• Complete health history and admission processes, identifying needs and appropriate referrals
• Provide direct patient care and personal care with compassion and clinical competency
• Identify discharge planning and education needs, and deliver patient/family education to optimize outcomes
• Communicate patient progress clearly with patients, families, and the multidisciplinary team
• Implement nursing interventions and prescribed medical regimens safely and document care accurately
• Delegate tasks appropriately and collaborate to modify plans of care in partnership with licensed practitioners
• Recognize early warning signs of patient condition changes and report them promptly

Qualifications

• Active MN LPN Licensure required
• Demonstrated ability to perform patient assessments and maintain accurate documentation
• Experience in discharge planning, patient education, and basic nursing interventions (as applicable)
• Strong clinical communication skills and ability to collaborate with multidisciplinary teams
• Capability to delegate tasks safely and work independently within scope of practice
