Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents and patients
Administer medications and treatments accurately and safely
Monitor and document vital signs, changes in condition, and care provided
Collaborate with the interdisciplinary team to create and follow care plans
Support ADLs (activities of daily living) and promote patient comfort and dignity
Communicate clearly with patients, families, and team members to ensure continuity of care
Maintain accurate, timely charting and follow facility policies and procedures

Qualifications

Current LPN license in good standing (Pennsylvania licensure preferred)
Graduation from an accredited practical/vocational nursing program
Basic Life Support (BLS) certification or willingness to obtain
Strong clinical judgment, compassion, and effective communication skills
Reliable, professional, and able to work collaboratively in a fast-paced environment

Education

Diploma or certificate in Practical Nursing (LPN/LVN) from an accredited program
Additional certifications (e.g., BLS) as required by facility

Knowledge & Skills

Medication administration and documentation
Basic wound care and infection control practices
Familiarity with electronic charting and clinical documentation
Ability to assess patient status and escalate concerns appropriately
Time management and prioritization in a busy care setting
