Contract Type: Per-Diem

Shift Type: 8-hour shift (exact start/end times and days to be determined) - Day shift preferred; schedule to be confirmed with facility

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care according to care plans
Administer medications and treatments safely and accurately
Monitor patient condition and report changes to the care team
Document patient care thoroughly and maintain accurate records
Collaborate with nurses and interdisciplinary staff to support quality outcomes
Assist with activities of daily living while promoting patient dignity and independence

Qualifications

Current, active LPN license in good standing (state as required)
Basic Life Support (BLS) certification or willingness to obtain
Comfortable performing medication administration and basic clinical skills
Strong communication skills and a patient-centered attitude
Previous experience in long-term care or skilled nursing is a plus

Education

Completion of an accredited LPN/LVN program
High school diploma or equivalent

Knowledge & Skills

Understanding of infection control and patient safety practices
Ability to chart accurately and follow care plans
Strong clinical observation and reporting skills
Teamwork and interpersonal skills to work with residents and staff
Time management and the ability to prioritize patient needs
