Roles & Responsibilities

Provide compassionate, high-quality bedside care to residents, ensuring dignity and comfort
Administer medications and treatments accurately, following facility protocols
Perform thorough patient assessments and document observations clearly and promptly
Collaborate with the interdisciplinary care team to create and follow individualized care plans
Support activities of daily living and promote resident independence whenever possible
Communicate effectively with residents, families, and staff to ensure coordinated care

Qualifications

Current LPN license in Pennsylvania (must be active and in good standing)
Valid BLS/CPR certification (or willing to obtain upon hire)
Comfortable with medication administration, wound care basics, and routine nursing tasks
Ability to pass background check and meet facility immunization requirements
Strong teamwork, communication, and patient-centered care mindset

Education

Practical Nursing diploma or equivalent (LPN)

Knowledge & Skills

Solid knowledge of clinical nursing fundamentals and medication administration
Skilled in accurate charting and documentation (electronic or paper charting)
Familiarity with infection control practices and resident safety protocols
Good assessment skills and the ability to prioritize tasks in a busy environment
Strong interpersonal skills for working with residents, families, and care teams
