Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications accurately and document in the chart
Monitor and record vital signs and report changes promptly
Assist with activities of daily living and support patient comfort
Collaborate with nurses, physicians, and the care team to deliver coordinated care
Maintain accurate, timely documentation and follow infection-control practices
Educate patients and families in a clear, reassuring way

Qualifications

Active LPN license (practical nurse) — Pennsylvania licensure preferred
Completion of an accredited practical nursing program
Current BLS (Basic Life Support) certification preferred
Comfortable with medication administration, wound care basics, and patient assessments
Strong communication skills and a team-oriented mindset
Previous experience in a long-term care or skilled nursing setting is a plus

Education

Graduate of an accredited practical nursing (LPN) program
High school diploma or GED

Knowledge & Skills

Patient assessment and clinical observation
Medication administration and documentation
Basic wound care and infection prevention
Electronic health record (EHR) charting
Time management and prioritization in a busy care setting
Empathy, clear communication, and teamwork
