Roles & Responsibilities

• Provide compassionate direct patient care and daily clinical support to residents
• Administer medications and treatments accurately and document per policy
• Monitor patient status, recognize changes, and escalate appropriately
• Collaborate with the interdisciplinary team to develop and execute individualized care plans
• Perform and document assessments, vital signs, and routine clinical tasks
• Educate residents and families on care plans, medications, and safety measures
• Maintain accurate, timely charting in the electronic medical record

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Basic Life Support (BLS/CPR) certification (required or must obtain upon hire)
• Comfortable with medication administration, wound care basics, and routine nursing procedures
• Excellent communication and teamwork skills; compassionate bedside manner
• Ability to pass background check and meet facility immunization requirements
• Experience in long-term care, skilled nursing, or rehab settings is a plus

Education

• Graduate of an accredited practical nursing program
• High school diploma or GED required

Knowledge & Skills

• Strong clinical assessment and observation skills
• Medication administration and documentation accuracy
• Familiarity with infection control and resident safety practices
• Proficient in patient-centered care and care-plan execution
• Basic computer/EMR navigation and timely charting
• Ability to multitask and prioritize in a fast-paced care environment
