Shift Type: Rotating 12.5-hour shifts, schedule varies (specific start/end times set by facility)

Hours per Week: 36

Candidate Pay Rate: $37 per hour

Roles & Responsibilities

Provide compassionate, high-quality nursing care to residents as an LPN
Administer medications and treatments accurately and document care thoroughly
Monitor patient conditions and report changes promptly to the care team
Support ADLs and promote residents' independence and dignity
Collaborate with RNs, CNAs and interdisciplinary staff to deliver coordinated care
Maintain accurate, timely documentation in resident records and follow facility policies

Qualifications

Current LPN licensure in Pennsylvania (or compact/reciprocal license as applicable)
Graduation from an accredited practical nursing program
Current BLS (Basic Life Support) certification preferred or willingness to obtain
Demonstrated clinical competency and strong communication skills
Ability to work 12.5-hour shifts and adapt to rotating schedules

Education

Completion of an accredited Practical Nursing program (LPN)
High school diploma or equivalent

Knowledge & Skills

Practical nursing skills including medication administration and wound care
Patient assessment and documentation best practices
Comfortable working in long-term care / skilled nursing environments
Strong interpersonal skills and the ability to work well in a team
Time management and prioritization during busy shifts
