Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents, putting dignity and safety first
• Administer medications and treatments accurately and document them in the care record
• Monitor resident condition, record vital signs, and promptly report changes to the care team
• Collaborate with nurses, therapists, and support staff to create and follow person-centered care plans
• Maintain clear, timely documentation and ensure accurate charting in electronic records
• Support resident independence through daily living assistance and meaningful engagement
• Respond professionally in emergencies and participate in facility safety practices

Qualifications

• Valid LPN license in Pennsylvania (required)
• Current CPR/BLS certification preferred or must obtain upon hire
• Comfortable working in a long term care or skilled nursing environment
• Strong communication skills and a collaborative mindset
• Reliable, compassionate, and patient-focused attitude

Education

• Graduation from an accredited Practical Nursing program or equivalent LPN certificate/diploma

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic wound care and knowledge of infection prevention
• Familiarity with long term care routines and resident-centered care
• Proficient charting and electronic health record usage
• Good time management and prioritization in a fast-paced setting
• Strong teamwork and conflict-resolution skills
