Roles & Responsibilities

Provide compassionate hands-on nursing care to residents and patients
Administer medications and treatments accurately and safely
Monitor, record, and report patient vital signs and condition changes
Collaborate with nurses, providers, and care team members to coordinate care
Educate patients and families about care plans and self-care practices
Maintain accurate documentation and ensure compliance with facility policies

Qualifications

Active LPN license in Pennsylvania (required)
Current CPR/BLS certification or ability to obtain upon hire
Strong clinical judgment with attention to detail
Excellent communication and teamwork skills
Able to pass required background and health screenings

Education

LPN diploma or practical nursing certificate

Knowledge & Skills

Medication administration and basic wound care
Patient assessment and monitoring
Familiarity with infection control best practices
Comfortable using electronic health records and documentation systems
Ability to work effectively as part of an interdisciplinary care team
