Roles & Responsibilities

Provide high-quality, compassionate nursing care to residents and patients
Administer medications and treatments safely and according to protocols
Monitor patient status and report changes promptly to the care team
Maintain accurate, timely clinical documentation in the charting system
Collaborate with nurses, therapists, and support staff to coordinate care
Support resident comfort, dignity, and independence through daily care activities
Participate in shifts and handoffs to ensure continuity of care

Qualifications

Active LPN (Licensed Practical Nurse) license in Pennsylvania required
Current basic life support (BLS) certification preferred or obtain upon hire
Clinical experience in a long-term care, skilled nursing, or similar setting is a plus
Strong communication skills and ability to work effectively within a care team
Stable documentation and charting skills, plus attention to detail

Education

LPN diploma or vocational nursing certificate

Knowledge & Skills

Medication administration and safe handling
Patient assessment and basic clinical monitoring
Wound care and ADL support for residents
Electronic charting and documentation basics
Infection control practices and resident safety
Team collaboration and compassionate patient interaction
